Student Nutrition Forms must be completed and signed by every family (even families who do not plan to use the school meal service program should fill out one of these forms; just write “Not interested” across the form if you are not going to participate, and please don’t forget to write your student’s name on the form. 5th-8th grade: forms are attached to the Bulletin; K-4th grade: forms will be sent home with students. All Student Nutrition forms should be returned by Friday, August 19th.
